Absence Requests

FALL 2022

All Absences must be documented using the Absence Request Form. Forms must be filled for each date that will be missed. This applies to both planned and unexpected absences (sickness, etc).

If your student is sick or in the event of a family emergency please send an email to  Dave Shinault and fill out the Absence Request Form. 

If you have an event that will cause you to miss a Contest Performance in the Fall and have not communicated that with Mr. Muth please do so by May 23rd as well.
